As part of its 40th Anniversary Tour, the CNIB EYE VAN is coming to MANITOUWADGE, sponsored by the Manitouwadge General Hospital & Family Health Team.
- Are you experiencing problems with your vision?
- Do you have diabetes, glaucoma, macular degeneration or a family history of eye disease?
If you answered yes, please contact your doctor, optometrist, nurse practitioner or diabetes educator for a referral to the CNIB Eye Van.
You may also be screened for diabetes or have your blood sugar checked if you are diabetic. The Manitouwadge Diabetes Health and Wellness Program is offering diabetes education and screening services at the Eye Van location as part of the Ministry of Health and Long Term Care’s Ontario Diabetes Strategy.
Appointments on the Eye Van are limited. Patients who need routine eye examinations or prescription glasses should see an optometrist.
